Prescriptions Refills

Please bring an updated list of medications to all appointments. Prescriptions may be requested at your appointment, via phone, the patient portal, or your pharmacy. When leaving a phone message for a prescription refill, include your name (with spelling), date of birth, phone number, prescription name (with spelling), and dosage. Please allow 24 business hours for refill requests. Our office will contact you by phone ONLY if there is a problem with your request.

For refills on controlled prescriptions, you must come into the office for an evaluation by one of the providers.

Prescriptions will only be refilled during regular office hours.

Test Results
​
Imaging and lab results will be given at your next follow-up appointment. We do not routinely call patients with results unless there is an immediate concern.​
Lighthouse Primary Care will accommodate as many requests as possible in an accurate and timely manner. Blank forms will not be accepted. Personal information must be completed. Turnaround time for forms and letters is seven business days. Forms are only completed for those accounts that are in good standing. Outstanding balances need to be paid before forms are filled out. Many forms require a current examination before being completed. FMLA forms must be picked up at the office as they will not be faxed or mailed to you or your employer. Payments for forms and letters are due at the time of pick up.
