What are your office hours?

For your convenience, patients are seen during the following hours:

Office Hours are Monday-Thursday 8:00 am - 5:00 pm.

Patients are seen during the following hours:

Monday-Thursday 8:30 am - 4:30 pm.

Friday 8:30 am - 12:30 pm.

The office will be closed Monday - Thursday for lunch from 12:00 pm - 1:00 pm.

How do I make an appointment?

To make an appointment, call 302-674-4545.

Do you accept walk-ins?

We only accept walk-ins if the schedule permits. It is best to schedule an appointment.

What health insurance plans do you accept?

We will be accepting the following healthcare insurance plans:

  • Aetna

  • AmeriHealth Caritas

  • Delaware First Health

  • Delaware State Medicaid

  • Highmark Health Options

  • Highmark BCBS

  • Medicare

  • Tricare

  • United Healthcare

What if I don’t have insurance?

We accept bank debit cards, major credit cards, and cash payments and offer Care Credit for healthcare services.

Will I be charged for missing an appointment?

Patients will be charged a $50 no-show fee for each appointment that is missed and/or not canceled within 24 hours. Patients will be discharged from the practice for three no-shows within a year.

What if I am late for my appointment?

Please arrive on time for your appointments. Understanding that situations and circumstances may arise, we allow a 15-minute grace period as long as our schedule permits, and if so, you may be seen later than scheduled. We must reschedule your visit if you are more than 15 minutes late.

How do I pay my balance?

Balances may be paid at the office or online here.

The following methods of payment are accepted:

  • American Express

  • Bank Debit Card

  • Care Credit

  • Cash

  • MasterCard

  • Visa

Can I be billed for my co-pay?

By Federal Law and Managed Care Contract law, this office is required to collect co-payment and deductible for each encounter. Penalty for not following this requirement could result in the termination and cancellation of medical coverage for the patient. Our agreements with health insurance plans require us to collect co-pays and deductibles at the time services are rendered. The Center for Medicare and Medicaid Services requires that we collect co-insurance and deductibles from Medicare beneficiaries. Please help us comply with these requirements by making your co-pays.

Can I schedule a Workers' Compensation visit?

We will not see patients with work-related injuries. Please get in touch with your human resources department for assistance with these cases.

What do I bring for a Motor Vehicle claim?

Before being seen for motor vehicle injuries, the following information must be presented:

  • Motor Vehicle Insurance Name & billing address

  • Claim Number

  • Insurance Adjusters Name

  • Insurance Adjusters Phone Number

If the above information is not presented before being seen by the provider, the patient will be responsible for paying the entire bill in full at the time of service.

Will you call me to review my testing results?

Imaging and lab results will be given at your next follow-up appointment. We do not routinely call patients with results at this time unless there is an immediate concern.

Will you complete my medical forms?

Lighthouse Primary Care will accommodate as many requests as possible in an accurate and timely manner. Blank forms will not be accepted. Personal information must be completed. Turnaround time for forms and letters are seven business days. Forms are only completed for those accounts that are in good standing. Outstanding balances need to be paid before forms are filled out. Many forms require a current examination before being completed. FMLA forms must be picked up at the office as they will not be faxed or mailed to you or your employer. Payments for forms and letters are due at the time of pick up.

Form Fees

  • Reviewing and completing medical forms is $30.00.

  • Medical letters are $25.00 each.

  • Physical forms presented during an exam are at no charge.

  • Physical forms presented after an exam are $15.

How do I get a copy of my records?

If you transfer care to another provider from Lighthouse Primary Care, we have a copier service to make copies of records here weekly. There is a fee to obtain records. Please call the office for pricing details.
