Lighthouse Primary Care is a nurse practitioner-led adult primary care clinic owned and operated by Sandra Eggleston, providing the primary healthcare needs of adults ages 18 and older. Primary care is broad in scope; however, our patients will receive comprehensive care ranging from health promotion and prevention to the treatment and management of acute and chronic illnesses for the person as a whole throughout the lifespan. Our mission at Lighthouse Primary Care is to provide the best primary care to our patients while treating each individual with dignity, respect, understanding, kindness, and courtesy.

DNP, APRN, FNP-C, CMOM

Sandra Eggleston was born and raised in Laurel, Delaware. In 2006, as a second career, she began working in healthcare as a direct support professional before becoming a certified nursing assistant. She continued to advance in her career by earning and working as an LPN and an RN in various healthcare settings. Sandra graduated from Simmons University with a master’s degree in nursing, becoming a Nurse Practitioner certified by the American Academy of Nurse Practitioners (AANP), and began working in private practice. Continuing her education, she earned a Doctorate Degree in Nursing Practice from Wilmington University, accomplishing her educational goals. To help fulfill the need to expand primary health care services in the community, Sandra opened Lighthouse Primary Care. 

 

Outside the clinic, Sandra is an active member of her church, serving in various ministries. She also enjoys spending time with family, reading, traveling, and relaxing at home.

AS, CMOM

Rebecca Delphin was born and raised in Miami, Florida. She has spent most of her career as a hotel general manager, where she was responsible for both the operation of the hotel and developing strategic plans for growth and profitability. Rebecca is now a certified medical office manager overseeing practice operations, personnel and financial management, compliance regulations, and managed care delivery systems. Rebecca is known for her strong work ethic, communication skills, commitment, and kindness toward everyone she encounters. She is dedicated to supporting the practice, providers, support staff, and patients and being able to serve the community.


Outside the clinic, Rebecca enjoys a good movie, volunteering at church, taking long walks, cooking, and writing inspirational poetic expressions.

Workplace Violence

Our office has a ZERO-TOLERANCE policy for workplace violence, disruptive behavior, verbal and nonverbal threats, and related actions.

Missed Appointments

Please give our office at least 24 hours notice when you need to cancel an appointment so that we may offer it to someone else in need. Patients will be charged a $50 no-show fee for each missed and/or not canceled appointment within 24 hours. Patients will be discharged from the practice for three no-shows within a year.

New patients will be discharged for not showing up for their first appointment.

Late Arrival to Appointments

Understanding that situations and circumstances may arise, we allow a 15-minute grace period as long as our schedule permits. If so, you may be seen later than scheduled. It's best to arrive on time for your

appointments. We must reschedule your visit if you are more than 15 minutes late. 

Payments

Patients must pay their co-pays and account balances at the time of service. The following methods of payment are accepted:

  • American Express

  • Bank Debit Card

  • Care Credit

  • Cash

  • MasterCard

  • Visa

Cell Phones

PLEASE SILENCE YOUR CELL PHONE WHILE IN THE OFFICE, as cell phone use can sometimes interfere with communication between the patient, provider, or our staff, which may cause distractions, misunderstandings, or delays in care. We request that you silence your cell phone upon arriving for your appointment. We appreciate your cooperation.

Other

Our office does not fill out tint waivers.

Prescriptions will only be refilled during regular office hours.

Insurance Coverage

Lighthouse Primary Care will file insurance claims on your behalf for insurance that we participate in; however, it is your responsibility to know the benefits covered by your policy. This includes co-pays, co-insurance, deductibles, and non-covered services. Please remember your insurance agreement is between you and your insurer. You are financially responsible for any charges generated in this office that are not covered by your insurance. Please get in touch with your insurance carrier directly for any questions regarding your coverage by dialing the member services number on your insurance card.

To submit claims to any insurer, we must have a copy of the insurance card, the policyholder's name and relationship, complete patient information, and other information as necessary. If we do not have sufficient information to process a claim, you will be responsible for payment at the time of service.

We will be accepting the following healthcare insurance plans:

  • Aetna

  • AmeriHealth Caritas

  • Delaware First Health

  • Delaware State Medicaid

  • Highmark Health Options

  • Highmark BCBS

  • Medicare

  • Tricare

  • United Healthcare

Referrals and Prior Authorizations

Your insurance carrier has established guidelines for referrals and prior authorizations. New referrals usually require an evaluation by your provider before approval; therefore, based on your insurance carrier, ensure you have a referral before seeing a specialist.

Testing ordered by your provider may require prior authorization. Not all testing will be authorized, and your provider may need to order a different test per your insurance company's guidelines. Some medications require prior authorization, which may take up to two weeks. Please note that medications may not get approved and may need to be changed to one covered under your plan. It is important to be familiar with your insurance guidelines.

No Insurance

We accept bank debit cards, major credit cards, and cash payments and offer Care Credit for healthcare services.

 

Workers' Compensation

We will not see patients with work-related injuries. Please get in touch with your human resources department for assistance with these cases.

 

Motor Vehicle Claims

Before being seen for motor vehicle injuries, the following information must be presented:

  • Motor Vehicle Insurance Name & billing address

  • Claim Number

  • Insurance Adjusters Name

  • Insurance Adjusters Phone Number

 

If the above information is not presented before being seen by the provider, the patient will be responsible for paying the entire bill in full at the time of service.
